Before Your Appointment

Our front desk will send a text or email reminder with location, parking, and directions. - Gather Medical Records
Include hospital discharge summaries, imaging (MRI/CT) discs, prior EEG reports, and notes from any previous neurologists or pediatricians. - Medication List
Bring an updated list of current prescriptions, dosages, and supplements. - Insurance and Photo ID
Have your current insurance card and government-issued ID ready for verification. - Completed Patient Forms

During Your Visit
You will meet with one of our neurologists to discuss medical history, symptoms, and concerns. Depending on your needs, diagnostic testing such as EEG or MRI coordination may be scheduled.
